ABOUT THE SESSION
Personalised Care underpins the NHS Long Term plan for the future of Health and Care Delivery. Evidence shows that a personalised approach improves outcomes for service users, enabling them to live healthier lives for longer, whilst reducing costs for the system.

Do you need support to develop your workforce knowledge, skills and confidence in #PersonalisedCare and Personal Health Budgets?
Join other system leaders and influencers to come together and empower the North East and North Cumbria (NENC) Health and Care Integrated Care System (ICS) to lead on #PersonalisedCare.
This conference will showcase the #PersonalisedCare work currently being undertaken by all sector partners in North Cumbria and the wider region. It will inspire and support you to produce personal and team goals, whilst empowering you to lead and influence others to improve the care of our service users. The event will give opportunity to network with the teams delivering #PersonalisedCare, highlighting roles, case studies and how services are embedded across the region.
Leaders in #PersonalisedCare from across the region will explain the project so far, strategic overview, data, related policies and evidence to show how the work looks from a system view. Keynote speaker will deliver presentations on how #PersonalisedCare relates to population health and how lived experience can impact service delivery and improved outcomes. There will also be workshops on topics such as ‘Green Social Prescribing’, ‘Embedding shared decision making into practice’, ‘Partnership working’ and ‘Peer leadership’.
AIMS:
Engage and inspire NENC ICS leaders and influencers.
Encourage continued funding and engagement for the work around personalised care, collaboration and coproduction.
Ensure further collaborative working with ALL partner organisations across the ICS
Encourage coproduction throughout service design and delivery.
Embed #PersonalisedCare workforce development across system.
Encourage and embed a #PersonalisedCare approach in practice therefore improving outcomes and reducing health inequalities.
