DESCRIPTION:Population Health Academy Best Practice October Event\n\nOne of
  our ICB health inequality clinical leads and local GP\, Dr Azhar Fareed\,
  will be talking about the population health approach taken by his GP prac
 tice in Central Lancashire with a cohort of patients with diabetes/pre-dia
 betes. The cohort was a group of South Asian women and the approach took a
 ccount of cultural needs to facilitate better engagement with diabetes pre
 vention.\n\nEveryone is welcome that has an interest in population health\
 , reducing health inequalities and working to improve health outcomes and 
 reduce inequity.\n\nPlease join us for what promises to be both an interes
