  charity.\n\nOn 19th Nov Pathway will be hosting a free webinar for NHS c
 ommissioners\, local authority and health service leaders about how to imp
 rove outcomes for your homeless population\, save money\, and shift demand
  from hospital to community.\n\nJoin us to find out how you can:\n\n 	Impr
 ove outcomes in acute and mental health Trusts through an evidence-based a
 pproach to better management of homeless patients.\n 	Give a more effectiv
 e\, safer response to patients who are homeless\, and prevent people being
  discharged back to the street.\n 	Generate wider system benefits and savi
 ngs\, including moving care from acute settings to the community.\n 	Use P
